Blessing Health System has partnered with Biofourmis, a company specializing in virtual care and digital therapeutics, to launch a new Home Hospital program. This initiative allows Blessing to provide acute-level hospital care to patients in their homes, supported by Biofourmis's technology solution. This program is part of the Rural Home Hospital project and aims to deliver care for a range of acute conditions outside of a traditional hospital setting. The goal is to optimize hospital capacity for more critically ill patients, potentially leading to higher patient satisfaction, lower readmission rates, fewer hospital-acquired conditions, and reduced overall care costs. For rural communities, this program also helps lessen the burden of travel for patients and their families. Blessing providers will conduct home visits and use Biofourmis technology, including biosensors and a patient app, for remote monitoring. Blessing Health System is among the health systems nationwide participating in the Acute Hospital Care at Home waiver, which provides Medicare reimbursement parity for eligible patients receiving acute care at home. Since continuing its Hospital from Home program after a research trial, Blessing has cared for over 300 patients in their homes, reporting high patient and provider satisfaction and quality outcomes comparable to or better than in-hospital care. Blessing continues to develop its mobile and telehealth strategy to potentially expand this program to include more complex conditions in the future. [4, 3]
