ElderONE is a Program of All-Inclusive Care for the Elderly (PACE) designed to help older adults (55+) with chronic care needs live safely and independently in their own homes and communities. [1, 2, 6] They provide a comprehensive range of medical, social, and support services including medical care, prescription drugs, transportation, adult day care, rehabilitation services, acute hospital care, in-home nursing and personal care, durable medical equipment, nutritional counseling, meals, and medication management. Primary Electronic Health Record System
ElderONE, which is part of Rochester Regional Health, utilizes the MyCare patient portal. This patient portal is connected to Rochester Regional Health's electronic health record (EHR) system. Rochester Regional Health has partnered with Cerner, a healthcare technology company, for their new EHR system, and MyCare serves as the patient portal for this Cerner EHR. Therefore, similar to how Epic is tied to MyChart, Cerner is the EHR vendor associated with the MyCare patient portal used by ElderONE through its affiliation with Rochester Regional Health.
