About the Practice

Damian Family Care Centers, Inc. is a not-for-profit Federally Qualified Health Center (FQHC) providing comprehensive primary care, dental care, and mental health services. They are committed to serving all patients regardless of their ability to pay, utilizing a sliding fee scale based on federal poverty guidelines. Services include general primary care for all ages, HIV primary and specialty care, family planning, GYN services, pediatric and adult dental care, mental health counseling, GI/hepatology, podiatry, optometry, nutritional and health education, case management, outreach, eligibility assistance, and transportation services.
