Comprehensive Care Management (CCM), also known as Comprehensive Care Management Corporation (CCMC), is a provider under the Program of All-inclusive Care for the Elderly (PACE) in New York State. This program focuses on providing comprehensive long-term care services to frail elderly individuals aged 55 and older who are medically eligible for nursing home care, enabling them to live in their communities. Services include social day health, home health care, primary and subspecialty medical services, hospitalization, nursing home care, housing, and intensive case management to coordinate care and manage costs.
