The Pharmacy and Therapeutics (P&T) Committee approved updates to Clinical Criteria applicable to the medical drug benefit for Anthem. These policies were developed, revised, or reviewed to support clinical coding edits. This notice is to inform providers of new or revised criteria adopted by Anthem and applies to plans offered by AMH Health, LLC., a joint venture between MaineHealth and Anthem.
